5-0 out of 5 stars A diverse and thoughtful engagement of a multi-sided issue
Collaboratively compiled and edited by Charles Levin, Allannah Furlong, and Mary Kay O'Neil, Confidentiality: Ethical Perspectives And Clinical Dilemmas is comprised of a selection of essays by learned authors concerning the ethical, legal, and clinical issues regarding confidentiality in the relationship between psychoanalyst and patient. In the twenty-first century, an era in which personal privacy is increasingly stripped away, the importance of confidentiality in legal and psychoanalytical terms requires close scrutiny in a striving to balance the needs of the public at large with the rights of the individual. Case studies of ethical dilemmas, the consequences of breaches of confidentiality, problems directly relating to clinical practice as well as more general legal matters, and much more are covered in this diverse and thoughtful engagement of a multi-sided issue. ... 4-0 out of 5 stars Authoris on target
I have only read the synopsis of the book and the customer reviews. I have 11 years experience as an Industrial Nurse Case Manager. I wholeheartedly assure the secondary gain issues listed are quite true from 3rd party liability cases, workers' compensation cases and medical malpractice. There are a lot of unethical physicians who will causally relate anything to everything, continue treatment when none is warranted, (one case of mine had physical therapy for 15 years and never improved) and keep persons out of work for no objective reason. This is blatant fraud. Other physicians may innocently do this because they lack knowledge of secondary gain and/or symptom magnification. I try to stay away from this type as well as the hired gun physicians who do reap the financial benefits from insurance companies and are not objective either. The plaintiff attorneys coach their clients to drive up medical costs for increased settlement. All claims are greatly increased when pre-existing psychological conditions/disorders (i.e. depression, anger issues, borderline personality disorder) are present. There is a very significant direct cost to the taxpayer (i.e. federal workers' compensation cases, such as postal workers, shipyard workers, any civil service, etc.)and indirect cost, by our increased purchase costs due to the burden of risng insurance premiums which the company or individual must pay. Thus this affects all of us in a very direct way both monetarily and psychologically (decreased co-worker morale when they see a co-worker getting away with this, increased false claims, again because others see how easy it is for false claims, and anxiety caused by job loss when the company closes because they can no longer afford their premiums). I also think other factors color this situation as the general population's work ethic has spiraled downward while their sense of entitlement has sharply increased. This is the real world, not embellished opinion.

4-0 out of 5 stars conflict of interests, anyone?
I would rather prefer that those denying the science behind Dr Malleson's statements would declare their conflict of interests, as it is usually done in scientific papers. Maybe we could have a broader view about their opinions.

5-0 out of 5 stars an excellent overview
This book clearly illustrates the true nature of whiplash injuries - as a vehicle to help ne'er-do-wells exist without actually contributing anything to society.Read more

Editorial Review

Introduced in 1962, silicone breast implants had been used by between one and two million women by the time they were virtually banned in 1992 by David A. Kessler of the Food and Drug Administration. While the ban came about because manufacturers had failed to demonstrate the safety of the devices, Marcia Angell, executive editor of Read more

4-0 out of 5 stars Science Triumphs
I read this book back in 1996 before the issue of implant safety was settled.I recall the impression it made on me with regard to the widespread ignorance that surrounds us, not only on specific scientific principles but also on the whole idea of the scientific method. This book illustrates why hypotheses and theories can never be evaluated by a court of law.It also illustrates why in any critical review of data one must have a control. If the incidence of a disorder is the same in a group getting implants as it is in a group that does not get implants then it is impossible to reasonably argue that there is any connection between implants and the disorder.Silicone is one of the most inert materials around and people are exposed to it all the time.I strongly recommend this book for anyone looking for further proof that our schools do not teach science adequately.

5-0 out of 5 stars Medical evidence vs court testimony vs belief
Please note that the more or less average "average ratings" come from reviewers who either rate this one star or four to five stars. In my opinion this illustrates the main point of the book, i.e. that people either evaluate things based on medical evidence, on the opinion of experts, or on their personal belief based on personal life experience/expectations.The different ways of evaluation produce conflicting perceptions, conflicting world views which we see played out many aspect of life.I think that Dr. Angell is gutsy to even take on an issue which generatestremendous controversy.I believe that she presents the arguments for medical evidence very well. Chapter 5 is the best brief and understandable description of what medical evidence is (and is not) that I have ever read.I recommend the book on that alone.I hope that some day Dr. Angell will update the book to include more recent research.

1-0 out of 5 stars Biases Aplenty
For the true story behind implants, one's time is better spent reading John Byrne's "Informed Consent".He was in charge of Dow's "ethics" program and his wife also had implants.Contrary towhat Dr. Angell writes in her book, there are many studies showingimmunological responses to silicone oils (most recently a study byresearchers at the FDA - June 1999).The Institute of Medicine in theirrecent report termed the FDA study "interesting", choosing torely on previous, and seriously flawed, studies purporting to show no linkbetween silicone and disease (the same studies quoted in Dr. Angell'sbook). One of the studies looked at women who had implants for only onemonth (!), which is as ludicrous as determing smoking doesn't cause cancerafter studying a person who smoked for only a brief time. There arethousands of court documents proving fraud on the part of the implantmanufacturers.Reading through them makes one realize how powerfulcorporations are and how the bottom line is far more important than thehealth and safety of the consumer.Breast implants (saline and silicone)have NEVER been approved for safety by the FDA.They only have pre-marketapproval, which means they can be on the market pending the manufacturerssubmission of safety data.To date, the manufacturers have been UNABLEtoprove implants safe.Unfortunately, the medical community and thecorporate community are oftentimes inextricably intertwined.Money is themotivator. When Dr. Angell speaks of greed, I'm sure that's at least onething she knows a lot about. ... 5-0 out of 5 stars Very Useful and Informative
I presently use this book as a text for a course for Imaging Systems students. They have found it easy to read and informative. While it dwells on the legal aspects quite heavily, it still delves into the personal aspects of ethical issues which might be encountered by imaging professionals. ... Read more

5-0 out of 5 stars Another breath of fresh air
Consider the following scenario: a married couple is told by a government official that they will not be allowed to have sexual intercourse since the woman among them may get pregnant. Sexual reproduction is very hazardous for embryos, fetuses, and gestational mothers they are told, and the statistics supporting the dangers in procreation is presented to them. The couple takes the statistical data to a trained statistician in order to obtain an opinion on the reliability of the data. The statistician informs them that the data is correct, including the statement that up to 75 percent of embryos conceived through sexual intercourse never make it birth. Most of the embryos do not implant in the uterus and are spontaneously aborted. The couple, because of the current legal environment that forbids behavior that leads to these kinds of dangers, is therefore prohibited from procreating using traditional intercourse.
5-0 out of 5 stars Defense of the Clones
This book is superb. The writing is very clear and the restatement, at convenient intervals, of the points previously established is very useful. It gives a comprehensive, thoughtful and balanced account of the scientific as well as the ethical and legal issues concerning human reproductive cloning.
5-0 out of 5 stars A Case Study in Arguing Before a Judge and Jury
Prof. Kerry Macintosh of the Santa Clara University Law School, who got her JD from Stanford, provides us with an excellent resource. Her book reads like a scholarly Ph.D. Thesis in addition to being a valuable case study on how to argue persuasively before a judge and jury. She supports her argument that anti reproductive-cloning laws passed in a number of states are unconstitutional and reinforce false stereotypes that stigmatize potential human clones as subhuman or unworthy of existence!Actually, if a clone were born healthy, he/she would really be nothing more than a "temporally displaced identical twin (say, by 40 years)," and twining (both identical and fraternal) is really nothing new in human history, is it?This seeming paradox is related to the "identity fallacy" that assumes that clones are somehow "more" than identical twins (actually they're less, since they had different wombs and maternal mtDNA).Take for example the false logic of the following argument:"It would be unethical to clone King Tut, since the world has changed so much since his day; how could he possibly fit in?"Really, it seems that our evangelical legislators fear that meddling in the "secrets of life," something which is only the proper dominion of God, will unleash unimaginable horrors.
